- For a current rate chart, click here to find the monthly premium based on your age, gender, product choice, and NJ county of residency. (As an example, the premium for a 43 year-old single female living in Bergen County who chooses the Basic and Essential EPO plan would be $261.79 per month.)
- To apply via mail or fax, click here to download an enrollment form. Note: you must indicate your method of payment on page 4 of the application (Letter I) and include a check, payable to Horizon BC/BS NJ or a credit card number, for the first month’s
payment. Your application cannot be processed if the first month’s payment is not included with your application.
